Submitted information is used for preliminary consultation, contact, treatment planning, travel coordination, and follow-up. It is not used as an automated medical diagnosis. Access should remain limited to the person handling international inquiries and authorized clinical personnel when a professional review is needed.
Medical photographs
The initial assessment form does not request facial photographs or sensitive medical documents. Do not submit photographs you are not authorized to share. The team will explain the designated private channel for any later clinical images.
Your choices
You may contact the international team to request access, correction, or deletion of the information you submitted, or to withdraw consent for future use. Information should be retained only for as long as reasonably necessary for the consultation and follow-up purpose.
